Remake Policy for Dentists
The cost of fabricating a custom made dental appliance cannot be refunded. Westdent Dental Laboratories may issue a credit directly to an account when an appliance(s) does not meet Westdent’s exceptional standards for quality, function and aesthetics.

Remake Policy

All remakes will be at no charge if received within 30 days of the invoice date EXCEPT under the following circumstances:

1. Westdent requested a try-in, however, the dentist/lab declined and asked for a completed case.

2. There is a shade or product change different from the original request.

3. The Partial Denture fits the master cast or there is a difference in original impressions.

4. Partials made over fresh extractions.

5. Partial Denture design is changed.

6. Westdent requested new impressions, however, the dentist/lab declined and asked for a completed case.

7. Relines on newly fabricated appliances will be processed at no charge only if received within 10 days, with the EXCEPTION of an Immediate appliance (charge will apply).

For eligible remakes within 30 days without the original appliance(s), Westdent shall remake the appliance(s) at 100% of the retail price of the restoration at the time the request is made. If the original model(s) and appliance(s) are returned within 60 days from the original invoice date, Westdent will issue a 100% credit to the respective account.

Cases canceled after fabrication was started (fabrication begins the day Westdent receives the case)will be billed for any work processed at time of cancellation.

Any credit balance on a Westdent account must be used for lab services within 60 days from the date of issue or it will expire.

Warranty of Work for Dentists
Westdent guarantees its work for one year against defects in material. The following appliances and services, however, are not covered by any warranty, because of their temporary or unique nature:

* West-Temps
* Stayplates
* Immediate Dentures
* Partials made over fresh extractions
* Rush/shipping fees
* TCS/Valplast (see below)

Westdent guarantees TCS/Valplast for life against breakage. Normal wear and tear however is excluded. In order to maintain natural luster with any removable restoration we fabricate, it is recommended to clean daily.

Repairs, relines, West-temps, immediate dentures and partials and appliances partially fabricated or completely fabricated by another laboratory other than Westdent Dental Laboratories, Inc. will not be covered under warranty.

For warranties to be valid, the respective Rx form must be properly completed with accredited signature.

Westdent Dental Laboratories,Inc. is not liable for any lost, damaged or stolen package shipped to you via a courier service. Shipping fees will apply regardless.

This warranty is in lieu of all other warranties, whether expressed or impllied and may not be modified by any agent, employee or representative of Westdent Dental Laboraotires, Inc.

Terms & Policies for Dentists
Terms:

Payment in full 20 days from date of statement. A 2% charge per month will be charged on past due accounts. Orders on past due accounts will be delivered COD with past due balance added. All cases and items sent, remain the property of Westdent Dental Laboratories, Inc. until client's account is paid in full. A minimum of $25.00 will be charged for returned checks. All disputes shall be governed in all respects by California law with venue in Los Angeles County, with the prevailing party to recover attorney's fees, court costs and other expenses, including actual expert witness fees, if any, in addition to any other relief to which prevailing party may be entitled.

Cases on hold for 60 days or more will automatically be billed to your account according to stage.

Remakes/Credits for Labs
Remakes/Credits:

The cost of fabricating a custom made dental appliance cannot be refunded. Westdent Dental Laboratories may issue a credit directly to an account when an appliance(s) does not meet Westdent’s exceptional standards for quality, function and aesthetics.

If the Customer requests a remake, the Customer will incur 100% or 50% of the cost for remakes processed by Westdent depending on the nature of the remake, under any circumstance. Credits requested without a remake are subject to evaluation. Terms and conditions:

I. If Customer’s RX instructions are not followed by Westdent’s technicians, the Customers’ case will be evaluated and subject to a remake credit on their account. The appliance subject to a credit must be returned to Westdent and remains the property of Westdent if credit applies. Remake Credit claim must be submitted in writing within thirty days from date of invoice.

II. If only a Credit is requested by the Customer without a remake, the case will be evaluated and subject to a credit on their account. The appliance subject to a credit must be returned to Westdent and remains the property of Westdent if credit applies. Credit claim must be submitted in writing within thirty days from date of invoice.

III. If RX states to fabricate our own design, Westdent is not liable to credit Customers’ account or incur the cost of a remake.

IV. Shipping and/or rush fees are not credited/refunded under any circumstance. These fees are the responsibility of the customer regardless if case is returned to Westdent for credit.

V. Any credit balance on a Westdent account must be used for lab services within 60 days from the date of issue or it will expire.

Terms & Policies for Labs
As of July 1, 2008, all new lab accounts are required to have a credit card on file with Westdent. Payment with case required for all new lab accounts. If payment is not included, the credit card on file will be charged for work completed and shipped. Net 30 will apply after 6 months of excellent payment history. A 5% charge per month will be charged on past due accounts. Orders on past due accounts will be delivered COD with past due balance added. All cases and items sent remain the property of Westdent Dental Laboratories, Inc. until client's account is paid in full. A minimum of $25.00 will be charged for returned checks. All disputes shall be governed in all respects by California law with venue in Los Angeles County, with the prevailing party to recover attorney's fees, court costs and other expenses, including actual expert witness fees, if any, in addition to any other relief to which prevailing party may be entitled.
